NYC Health + Hospitals celebrated Diabetes Awareness Month by highlighting the success stories of three patients who improved their diabetes through the Lifestyle Medicine Program. Patient Samuel Rios improved his A1C levels from 11.3% to 5.7%, patient Armando Perez Solis put his diabetes into remission with a reduction in A1C levels from 9.4% to 6.0%, and patient Davi Baez saw improvement in her A1C levels despite challenges. The interdisciplinary program offers tools for healthy lifestyle changes, such as counseling, group classes, and fresh produce deliveries.
Mayor Eric Adams championed the launch of the program in 2019, expanding it to six additional sites. Patients in the program have access to one-on-one counseling, weekly group classes, exercise sessions, fresh produce deliveries, and resources like Health Bucks for farmers markets. The program aims to empower patients to make evidence-based changes in nutrition, physical activity, sleep habits, stress management, and social connections. By addressing key root causes of type 2 diabetes, the program helps patients reclaim their health and vitality.
With over 3,000 patients enrolled since its inception, the Lifestyle Medicine Program is making a significant impact on the health outcomes of individuals living with chronic conditions. The success stories of Rios, Solis, and Baez showcase the transformative potential of lifestyle medicine in managing and improving diabetes. Through small changes and personalized care plans, patients are seeing tangible improvements in their health and well-being.
